Overview:
Submission of this form is required to review all new and unapproved voluntary and third-party training offered to and attended by faculty and staff. This form is also required for the review of all proposed department mission or value statements to ensure alignment with overall Institutional mission and values. This form must be approved by the CHRO and President to ensure the training or statements are in compliance with USG policies and in alignment with the institutions stated mission and values. Implementation or delivery of training is prohibited until approval is received.
